Cardiac Rehabilitation Pilot Program

Do you have upcoming heart surgery?
St Lukes has launched a pilot program to help our members adjust to life after hospitalisation for a heart related event.
What does the program involve?
The program runs for seven weeks and is delivered by Honeysuckle Health, on behalf of St Lukes, and is completed from the comfort of your own home.
The program is designed to support you as you recover, improve your understanding of your condition and adjust to your diagnosis. Cardiac rehab can improve your quality of life after a heart event and may reduce your risk of another heart related hospitalisation. The program includes:
- Weekly one-on-one phone calls with a registered nurse
- Access to the Cardihab mobile app to record and track your medications, blood pressure, symptoms, step count, stress, smoking, alcohol use and more
- Access to education articles and videos, resources, and wellbeing support
- Tailored exercise program to support your recovery and health goals
- Ongoing use of the app for 12 months

Am I eligible?
You may be eligible if:
- You currently hold a St Lukes Hospital cover that includes heart and vascular (Silver & Gold cover), have served all waiting periods and are financial;
- You have a compatible smart phone and reliable internet connection;
- You have been to hospital for an acute cardiac condition or procedure in the past 30-days; and
- You meet the clinical inclusion criteria after assessment by the Honeysuckle Health clinician including a cardiac rehabilitation referral from your treating cardiologist, specialist or GP if requested
